Common Medication Errors

2. Wrong Medication - The confusion arises due to similarly named drugs.
3. Wrong Patient - Administering medicine to the patient whose name does not match the prescription.
4. Wrong Route - Giving IV instead of oral or the other way round.
5. Wrong Time/Frequency – Missing doses or administering outside the scheduled timeframe.
6. Documentation Errors – A failure to document the medications that were given to the patient.

Prevention of Medication Errors

✅ Care With Labels And Read Carefully – Labels and prescriptions should be perused with due care before being administered to the patient.
✅ Manage Your Environment – Medication rounds should be carried out without any interruptions.
✅ Orders That Are Unclear Must Be Clarified – Orders that are vague or not clear should be checked with the doctors or pharmacists as they are capable of providing clarity.
